Living Options

Our living arrangements are tailored to each individual’s need. Whether a secure and monitored apartment is preferred or around-the-clock healthcare is required, we have inviting and pleasant accommodations.

Columbus Oaks Assisted Residential Living

Apartment style living with extra services allow our residents to remain in a home-like setting and includes their own furniture, cherished mementos and personal items. We promote independence and quality of life with 24-hour attendants, 24-hour emergency call system, three meals a day, medication assistance, housekeeping and laundry service, assistance with bathing, dressing and grooming, and scheduled transportation to doctor appointments, as well as shopping and recreational activities.

All assisted-living apartments include interior hallway access, private bathroom and walk-in shower, mini-kitchen, separate living and bedroom areas, individual temperature controls, and 24-hour emergency call system. Telephone and cable TV service is available for all assisted-living residents to enjoy.

We accept assisted living Medicaid waivers.

Columbus Oaks Certified Medicare and Medicaid Skilled Nursing Facility

Long-term Care

As an integral part of our community, we provide the professional care of registered and licensed nurses 24 hours a day. Medical services include skilled nursing, physical, occupational, and speech therapies, special diets and help with bathing and dressing. These services are delivered with care and compassion by our trained and competent staff.

Columbus Oaks skilled nursing facility accepts the following payer sources for long term care:

• Medicare
• Medicaid
• Private pay
• Most private/long-term care insurances

Short-term Care

Circumstances may require a short-stay in a skilled nursing facility and Columbus Oaks is ready to help those who have fallen, after a surgery requiring post-operative care or when an improvement in health and wellbeing is needed after a decline.

We have board-certified staff for physical, occupational and speech therapy, and we design individualized plans for optimum recovery. Columbus Oaks works with Reliant Rehabilitation, the nation’s largest privately owned rehabilitation company, who provide the highest quality rehab service to our in-house residents. Our goal with short-term rehabilitation is to get you or your loved one back to living life to the fullest.

We have private Medicare suites available. All Medicare suites have private bathrooms, phone service, and TVs equipped with cable at no extra charge. Columbus Oaks Healthcare Community is a licensed Medicare provider.

To qualify for Medicare’s Skilled Nursing benefit the following must be met:

• Individual was admitted to an acute care hospital for at least three midnights
• Must be admitted within 30 days of the discharge from the hospital or other skilled nursing facility
• The care and condition must require inpatient services, such as nursing, rehabilitation or both for treatment related to the original hospitalized condition

We also offer hospice and respite care. This facility has a specialty hospice unit, called “Cardinal Lane”, as well as a 28-bed secure unit for those with unique needs.
