St. Christina Nursing and Rehabilitation Center
After a person has surgery, a fall or just a decline in health, a boost may be needed to get back to their prior level of functioning. Our board-certified physical, occupational and speech therapists, along with our nursing team work together to create a customized rehabilitation plan while communicating frequently with family to discuss progress and to promote a positive experience. Our goal with short-term rehabilitation is to get you or your loved one back to living life to the fullest.
St. Christina Nursing & Rehabilitation Community is a licensed Medicare provider. To qualify for the Medicare Skilled Nursing benefit the following must be met:
- Individual was admitted to an acute-care hospital for at least three midnights
- Must be admitted within 30 days of the discharge from the hospital or other skilled nursing facility
- The care and condition must require inpatient services, such as nursing, rehabilitation or both for treatment related to the original hospitalized condition
St. Christina’s interdisciplinary team will assess and determine care needs in conjunction with physician guidance. This level of care is evaluated quarterly and upon a significant change in condition. We do not believe in a blanket approach to one’s care, each person is unique and the care plan will reflect this.
