Living Options

Cottonwood Creek is proud to offer three distinctively different lifestyle choices: skilled nursing, assisted living and memory care. Our fully trained staff along with our unique campus give residents convenience and a carefree lifestyle.

Assisted Residential Living

Apartment-type living with extra services allow our residents to remain in an environment that includes their own furniture and personal items to make the transition more comfortable and familiar.

The Assisted Residential Living program promotes independence and quality of life and includes three meals a day, help with medications, dependable maintenance, housekeeping, and laundry services, assistance with bathing and dressing and a 24-hour emergency call system.

These residential suites are available in several floor plans, from efficiency size (studio) to one bedroom/one bathroom units to suites with kitchens. TV and Wi-Fi are free and available for all of our assisted-living residents to enjoy.

We accept the assisted living Medicaid waiver.

Certified Memory Care

Bluebonnet Lane is a fully certified and licensed assisted-living memory care community, dedicated to improving the lives of those affected with memory loss and Alzheimer’s. Our goal is to provide an intimate residential setting so we can get to know each of our residents and customize our services to their individual needs.

Residents are encouraged to participate in activities which respect and support their strengths. Cottonwood is secured by a wooded courtyard with a continuous walking trail. We create opportunities for our residents to remain involved in the familiar routines that enriched their lives as well as give gentle reminders to assist them throughout their day. To help ease our resident’s complex journey, we strive to provide cognitive stimulation, reduce agitation, inspire creativity and instill a personal sense of purpose.

Dignity, respect, and an ongoing commitment to advanced memory care programs for our residents, coupled with peace of mind for their families, is the hallmark of care at Cottonwood Creek.

Medicare-Certified Skilled Unit

Board-certified physical, occupational and speech therapists work together with our nursing team to design customized rehabilitation plans. We communicate frequently with family to discuss progress and to promote a favorable experience.

We have private Medicare suites available. All Medicare suites have personal bathrooms and free cable television.

Cottonwood Creek Healthcare Community is a licensed Medicare provider. To qualify for Medicare’s Skilled Nursing benefit the following must be met:

• Individual was admitted to an acute care hospital for at least three midnights
• Must be admitted within 30 days of the discharge from the hospital or other skilled nursing facility
• The care and condition must require inpatient services, such as nursing, rehabilitation or both for treatment related to the original hospitalized condition

Nursing Center

Our Nursing Center provides the professional care of registered and licensed nurses 24 hours a day. Medical services include skilled nursing, physical, occupational and speech therapies, special diets, and help with bathing and dressing. These services are delivered with care and compassion by our trained and competent staff, assuring the needs of our residents are met, around the clock.

The Cottonwood Creek accepts the following payor sources for long term care:
